Closing the gap between medical care and psychosocial support
Shared Follow-up Appointments (SFA)
Workit Health’s Shared Follow-up Appointment (SFA) model integrates psychosocial care directly into medical visits.
Prior to implementation, fewer than 15% of members who attended medical visits also participated in counseling groups each month—despite access to more than 35 therapist- and peer-led sessions weekly.
Through SFAs, we are closing this gap and delivering more comprehensive, whole-person care.

Leveraging grant funds to lower barriers
With a federal grant to support expanding access to treatment in rural communities, we focused efforts on 17 counties in northern Ohio. In that region, we had:
- 200+ new residents enrolled in care
- 250% increase in rural residents receiving treatment since grant launch
- 69% retention at 3 months
- 95%+ satisfaction, with members preferring virtual care
150 uninsured or underinsured individuals had their treatment covered by grant funding across Michigan, Texas, and Ohio.
